How do I Obtain a Birth Certificate in Clay County, Florida?
- 1). Gather and copy the documents needed to obtain the birth certificate. You will only need a copy of a valid photo identification such as a driver license, passport, or military identification if you are ordering your own birth certificate or your child's birth certificate. You will have to provide a copy of the appointment orders if you are ordering a birth certificate for a child that you have legal guardianship of.
- 2). Download a copy of the Application for Florida Birth Record from the Clay County Health Department website or the Florida Department of Health website, depending on which agency you are ordering from (see Resources).
- 3). Complete the appropriate application by providing the registrant's name, date of birth, place of birth and parents' names. Provide your relationship to the registrant if you are not requesting your own birth certificate.
- 4). Mail the application, copy of photo identification and a check or money order for the application fee of the agency you are ordering the birth records from. If you are placing an order with Clay County, mail the documents and the $13 application fee to:
